“Requires a credit card on file before scheduling an appointment even with insurance. Told me she filed insurance and it was applied to my deductible. Come to find out she never filed the insurance as there was no record of a claim being filed. When I asked for the information of denial from the insurance, she would not send it to me and stopped answering my emails. I had to submit the claim myself.”
