Paying for psychiatric care — what insurance usually covers
Most health plans cover psychiatric visits and therapy, and federal parity law generally requires mental health benefits to be no more restrictive than medical benefits. Specialized treatments often need prior authorization, and a few remain mostly self-pay.
Usually covered
Core psychiatric care
Psychiatric evaluations, medication management, psychotherapy, and many telepsychiatry visits are covered by most commercial plans, Medicare, and Medicaid. Copays, deductibles, and network rules still apply.
Covered with prior authorization
TMS, SPRAVATO®, ECT, programs
Plans often cover TMS and SPRAVATO® for treatment-resistant depression once you meet their criteria, usually documented trials of prior antidepressants. ECT, PHP, and IOP programs commonly need approval too.
Usually self-pay
Off-label ketamine & some testing
IV, IM, and oral ketamine for mental health are off-label and generally not covered. Pharmacogenomic testing and some psychological testing are covered inconsistently. Ask for a written price first.
Checking your benefits before you book
- Call the member services number on your insurance card and ask about "outpatient behavioral health" benefits.
- Ask whether the clinic and the specific clinician are in network — psychiatry networks can be narrow.
- Ask whether you need a referral from a primary care provider.
- Ask about your copay or coinsurance for an evaluation versus a follow-up visit, and whether telehealth is billed the same.
- If the clinic is out of network, ask about out-of-network benefits and request an itemized superbill.
Getting TMS or SPRAVATO® approved
- Keep a list of every antidepressant you have tried, with doses, dates, and why each was stopped.
- Ask your prescriber to document your diagnosis and symptom scores in your record.
- For SPRAVATO®, confirm the center is in network and certified in the SPRAVATO® REMS program.
- Let the clinic submit the prior authorization — their staff do this routinely.
- If you are denied, ask for the written reason and use your plan's appeal process.
If you are paying out of pocket
- Ask for a written quote that lists every fee, including the initial evaluation and follow-ups.
- Ask whether the clinic offers a sliding scale, packages, or payment plans.
- Community mental health centers and federally qualified health centers often charge based on income.
- HSA and FSA funds can often be used for psychiatric care; confirm with your plan administrator.
