“I am a former patient. I have requested my medical records three times. The first time the receptionist says she can’t find me in the system. The second time I call she can’t find me in the system I ask to speak to a manager and am put on hold, the receptionist found my information and assures me it will be sent to my email. It was never sent to me. I call again this week and she can see my information and assures me she has sent my documents but they have not been sent. I am extremely disappointed in the service.”
