“I was very disappointed today. The medical assistant was very inattentive and messy. She did not take my blood pressure, temp or pulse rate, until Dr K, realized it had not been noted on the medical form. I was there for a DOT, physical. This form goes to the National Registry and it was missing my unit # and corrective lens to be checked. This was on the form from which the medical assistant should have been attentive too to transfer this information. She did do a new certificate to submit, now that is messed up , the street is spelled incorrectly when that wasn't an issue on the first certificate. I now have to go in person again, to get this correct and submitted for a 3rd time. Unacceptable. I have recommended this clinic but after today, I really dont know, one person created all this for not paying attention to detail.”
